I-9 Employment Eligibility Verification

NOTE: The newest version of the I-9 is now available. Employers must use this updated form dated 08/01/2023.

Federal Form I-9 is used to verify the identity and employment authorization of individuals hired in the U.S.

All employers are required to complete Form I-9 for each individual they hire or re-hire, including citizens and non-citizens. Form I-9 includes instructions for completion, as well as acceptable identity-establishing documents; these instructions should be carefully reviewed. Form I-9s must be stored separately from personnel files and must be retained for specific periods.

Reference Release Form

This Reference Release Form can be used to obtain reference verification of an applicant’s prior employment. It should be provided with application paperwork and then forwarded to the named previous employer to obtain the record of employment.

90-Day Evaluation Form

This form can be used by supervisors and employees for 90-day evaluations. Core competencies are divided into several categories with a quick rating system for ease of use. Includes comments and signature fields for both the supervisor and employee, acknowledging discussion and completion of the form for record keeping purposes..

Employee Warning Notice

Issues with employee behavior or work performance should be documented.  Documentation can support a future need for separation of employment.

Personnel Action Form

When there are changes to an employee’s personal or employment information, it is important that payroll personnel are notified of those changes (for example, an employee’s status being changed from non-exempt to exempt). This form can be used to record and notify the necessary personnel of important changes.

Payroll Deduction Authorization

This form can be distributed to employees to document payroll deduction authorizations.

Missing Punch Notification

This form can be used for non-exempt employees to report missing or inaccurate punches to supervisory staff. Documenting and correcting inaccuracies in time reporting is vital to keeping good payroll records and avoiding potential areas of liability.
