Employers can use this form to document an employee’s safety violation; a copy should be retained in the employee’s file.

Accident/Exposure Investigation Report

This form is for management to document an accident or exposure incident in the workplace.  Employers can use this form to record details of how the accident or exposure occurred, who was involved, and what actions were taken to correct the concern.

OSHA Job Safety and Health: It’s the Law

The federal OSHA Job Safety and Health: It’s the Law poster informs workers of their rights under the Occupation Safety and Health Act.  Covered employers must display the poster in a conspicuous location in their workplace.

    • Employers may wish to display a copy of the poster in a foreign language spoken by their workforce. The poster is available in Spanish, Arabic, Chinese, Haitian Creole, Korean, Nepali, Polish, Portugese and Vietnamese at the above link.

NOTE: Certain states have OSHA-approved State Plans and may require a different version of the poster to be displayed. Employers should check their local State Plan agencies for additional requirements. Click here for a directory of State Plan agencies.

Family and Medical Leave Act (FMLA) Poster

The federal Family and Medical Leave Act (FMLA) poster was recently updated in April of 2016.  However, the February 2013 poster can still be displayed to fulfill an employer’s posting requirement.

Fair Labor Standards Act (FLSA) Minimum Wage Poster

The federal Fair Labor Standards Act (FLSA) Minimum Wage Poster must be displayed by covered employers as of August 1, 2016.

  • Information on who the FLSA applies to can be found on the U.S. Department of Labor Wage and Hour Division’s (WHD) website. Click here to visit the WHD’s dedicated FLSA information page.

NOTE: Local or state minimum wage ordinances may precede the FLSA, and may require different posters to be displayed. Employers should check their local labor departments for additional requirements.

Employee Polygraph Protection Act (EPPA) Poster

Performance Evaluation Form

This form can be used by supervisors to evaluate an employee’s performance, then share the findings with the employee.  Includes comments and signature fields for both the supervisor and employee, acknowledging discussion and completion of the form for record keeping purposes.

Notification to Benefits Coordinator of New Employee

This form can be used by the hiring manager to notify a benefits administrator of a new employee. The new employee’s essential information is recorded to facilitate coordination of benefits and scheduling of an orientation.
