Voluntary Election Meal Period Waiver for 6 Hour Shifts (CA)

A California employee working more than five (5) hours per day must be provided a meal period of at least thirty (30) minutes. However, if an employee's total workday is no longer than six (6) hours, the employee and employer can mutually agree to waive the second meal period. Employers may use this form to establish such an agreement with an eligible employee.
