Voluntary Election Meal Period Waiver for 6 Hour Shifts (CA)

A California employee working more than five (5) hours per day must be provided a meal period of at least thirty (30) minutes. However, if an employee’s total workday is no longer than six (6) hours, the employee and employer can mutually agree to waive the second meal period. Employers may use this form to establish such an agreement with an eligible employee.

CRD-185-ENG / Sep 2022 – Sexual Harassment Fact Sheet

California employers are required to distribute an information sheet regarding sexual harassment to all employees. The CRD-185-ENG pamphlet, produced by the CA Dept. of Fair Employment and Housing, serves this purpose; alternatively, employers may develop an equivalent document that meets statutory requirements.

Note: this pamphlet fulfills the information sheet requirements, but does not act as a sexual harassment prevention policy, which all California employers are required to have.

DWC 9783 Notice of Predesignation of Personal Physician

California employees have the right to designate a personal physician outside the employer’s MPN whom they wish to be treated by in the event of an injury, as long as the employee provides written notice of their personal physician. This is an optional form employers may provide to employees wishing to predesignate a physician, if the employer does not develop their own form.

Notice to Employee – Labor Code Section 2810.5

Employers are required, at the time of hire, to provide employees written notice of certain employment and payroll information.

DE 2320 For Your Benefit: California’s Programs for the Unemployed

Provides information on UI, DI, PFL, and Job Service benefits available to the employee. This booklet must be provided to the employee at separation of employment.

CA DE 4 Employee’s Withholding Allowance Certificate

The DE 4 is used to compute the amount of taxes to be withheld from employee wages to accurately reflect state tax withholding obligations.

NOTE: This is not a required form; however, best practice is to present the form to new-hires in the event the new-hire wants to have a different withholding rate than the federal withholding.
