Emergency Contact Form Requirements (SB 294)

Requirement (Effective by March 30, 2026):
Employers must provide employees with the opportunity to designate an emergency contact.

Specific Requirement:
The form must include a direct question asking whether the designated emergency contact should be notified if the employee is arrested or detained at the worksite or during work hours.

Employers often maintain emergency contact information to support employees during unexpected events such as injuries, natural disasters, or other urgent situations. These forms allow employees to voluntarily provide the appropriate contact information should the need arise.

  • For California employees: Use the Emergency Contact Form.
  • For employees outside California: Use the Emergency Information Form.
