Promissory Note

Employers who offer loans to employees can use this form as a model for a repayment agreement. The sample forms offered below come in two versions:

• Payments - the employee indicates  the amount and date they will begin making monthly payments until the sum is paid in full.
• Withholding - the employee authorizes an indicated amount to be deducted from their paycheck until the sum is paid in full.
