A-4 Employee's Arizona Withholding Election

Employers must make Form A-4 available to employees at all times and to inform employees of the Arizona withholding election options. New employees must complete Form A-4 within the first five (5) days of employment; current employees should be provided Form A-4 if they wish to make changes to their withholding. This form should be maintained in conjunction with the federal Form W-4.
